Publicity Form

From time to time, the Banff/Canmore Speed Skating Club (BCSSC) publicizes the accomplishments of its skaters and associate members to the community through various means such as newsletters, displays, internet website and press releases for purposes, such as, but not limited to training athletes, training coaches, advertising, promotions and communications. In order to avoid any confusion or misunderstandings about privacy and use of personal information we ask permission from each member or the parent/guardian of a member who is a legal minor, to publish names, photographs and/or video images in these forms.

The BCSSC uses the club website to record times and other accomplishments of its members. This information is necessary for planning the ongoing development of each skater and for entry to meets, however it can also be fun for individuals to track their own progress. Each member has the option of restricting such information so that only they and members of the coaching staff and committee may see them.

Each member of BCSSC is asked to provide an email address for communications from the club and Speed Skate Canada. Whilst this email address is never disclosed to other parties, the club may provide a facility so that members can send email to each other through the website. Each member has the option to prevent such emails being sent.

Please complete the areas below indicating that you give permission to the BCSSC to use your name, photograph and/or video images, times/achievements and email address, in relation to the activities described above, during the speed skating season from 1 September 2009 to 31 August 2010.
